  Locating our Office:
 
The Menopause Center is located in the East Pavilion Medical Office Building at Providence St. Vincent Medical Center, Suite 219. That building is on the most east side (Portland side, not near the emergency room) of the medical center. Use the entrance by Catlin Gabel School, off Barnes Road. Do not come in by the emergency room entrance.

  Arriving on Time For Your Appointment!
 
Please allow 15-30 minutes of extra time before your appointment for parking and for us to get you ready for your appointment. This applies to new and returning patients. We need you in our office reception area before your appointment, not on the street or in the parking area.
 
  What to Bring:
 
Download and fill out our patient information forms before your visit. There are 3 forms to fill out and can be found here. Make sure you bring these forms with you to your first visit. Do not send them to us.
 
Be sure to bring your insurance card, if you have insurance. And, we need at least one photo ID.
 
If you have been sent a Menopause Guidebook, please read it before your Menopause appointment.
 
Be sure to give yourself extra time for us to get your chart prepared for your first visit. Please make sure you get to our office suite 15-20 minutes before your appointment time.

We do not provide specific patient information via this Web site. And we do not send or receive emails or text messages.
 
  Medical Records Transfer:
 
We prefer to arrange for the transfer of your medical records after your appointment, so we can assess what records we need, if any.
 
However, if you need to have some recent medical records sent to us before your appointment, please download and fill out a Records Release Form. You need to send it to the doctor who has the records you want.
 
Your medical records can’t be sent by verbally asking for the. You have to submit a written request.
